Needed in Fresno, CA for elderly healthcare. Will be responsible for providing occupational therapy services to participants/seniors. Travel may be necessary to support all other work sites. Duties include: Participating as member of interdisciplinary team (IDT) & completing initial, semi-annual, unscheduled, & annual assessments; Attending morning updates & reporting changes in participants' baseline status to appropriate staff on daily basis; Being involved in development & implementation of Quality Improvement (QI) activities; Performing intake screenings as assigned & making appropriate recommendations; Conducting initial comprehensive assessment in person, at requisite intervals, & developing plan of care for participants on-site, in hospitals, in nursing facilities, in assisted living facilities, & in their homes (evaluations include, but are not limited to: functional status, physical status, activities of daily living (ADL) status, cognitive functioning, visual-perceptual status, feeding skills, bed mobility, wheelchair needs, & home safety evaluations); Providing treatment as indicated; Demonstrating ability to utilize appropriate safety measures when providing care; Developing, instructing, & supervising other staff &/or family members on therapy program to be carried out by individuals other than occupational therapist. Required qualifications: Current Occupational Therapist license in State of California; CPR with First-Aid certification; Medically cleared for communicable diseases & has all immunizations up to date before engaging in direct participant contact; Min. 1 yr. documented exp. working with frail or elderly population; Ability to cope with mental & emotional stress related to position; Ability to function independently & to work effectively & in collegial manner with all members of interdisciplinary team, coworkers, & department heads; Ability to relate to public & governmental agencies in pleasant, patient, & professional manner; Well organized, dependable, flexible, & resourceful; Excellent communication skills. May require use of personal vehicle to provide therapy services in participant's residence. If using personal vehicle, valid CA driver's license is required. Send resume to [email protected].
